1. ACTIONS:
Dihydroartemisinin has strong schizonticidal action, but has no effect on exoery1hrocytic, sporogony and gametocyte fonns. Dihydroartemisinin, as a drug with biological action of Artesunate and Artemether, has been used for millions of patients. Active mechanism of Dihydroartemisinin has not been known fully. Some researches suggest that the drug concentrates selectively into cells contracted by parasites and reacts with hemin (hemozoin), to kill the parasites. This reaction produces poisonous free radicals that can destroy membranes of parasites in vitro.
Piperaquin, a derivative belongs to 4-aminoquinolin group, is a biquinolin which has the same action mechanism to other quinolins. Piperaquin had been used in China to replace Chloroquin and had paid a key role in prevention and treatment malaria in 1980s. Piperaquin has strong effect in ery1hrocyte fann of P.vivax , P.malariae and most strains of P.falciparum.(except gametocyte). LD50 of Piperaquin is 1098.5±78.2mg/kg (LD50 of Chloroquin is 427.9± 50.4mg/kg). Using Piperaquin is safe and not poisonous.
When using Dihydroartemisinin alone, the rate of recurring after stopping therapy is high. If using Piperaquin alone in prevention chloroquin-resistant P.falciparum, drug-resistance has appeared
Many recent researches confirmed that the safety and effectiveness of the combination of 2 above drugs in treatmenl of most malarias, not excluding malaria caused by multi-resistant P.falciparum in Vielnam. This combination has overcome the shortcoming of using the single drug. Basing on the experience of using Mefloquin, it is made known that the combination with Dihydroartemisinin can slow down the resistance against Piperaquin (the rate of failure in treatment malaria when using .Mefloquin alone is 50%; while combination with an artemisinin's derivative has good effect for 8 years (more than 95%)). In clinical trial of this combination, recurring has appeared lately. (perhaps due to Piperaquin's long latter half life)
2.INDICATION: Treatment of all kinds of malarias even malaria caused by multi-resistant Plasmodium falciparum

3. ADMINISTRATIONS AND DOSAGE:
Dosage:

Administration: first day divide the dosage into twice, the following days: once time daily,
Caution: To have high result, the drug should be taken intermittent 6, 24, 48 hours after the first dosage,
4. CONTRAINDICATION:
Patients with hypersensitivity to its components
5. SIDE EFFECTS:
Many patients were used dihydroartemisinin and piperaquin but there has not been recommendation about severe side effects, The side effects usually light and disappear when stop the treatment. The side effects are: on gastrointestinal (nausea, vomiting, stomachache, diarrhea), headache, dizziness and giddiness,
Inform doctor if any side effect occur when use the drug.
6. DRUG INTERACTION:
- Antacid: reduce drug's absorption
7. PRECAUTION:
- Take precaution when use the drug for pregnancy and lactation especially in first trimester, These cases have to have doctor's prescription, strict indications and observation,
- Should examine the eyes before the long treatment and must be observed through the treatment.
- Take precaution when use the drug for patients with renal, hepatic disease or porphyrin metabolism is confused, history of epilepsy,
- Stop use the drug if occurs severe confused blood,
